If you have received a sacrament at St. Gerard’s within the past 10 years, please provide the information requested on the Sacramental Records form below. You may submit the completed form by emailing it to [email protected] or by placing it in the collection basket or parish office. If you prefer, you may simply type the requested information in an email instead of using the form itself.
This includes baptisms, first eucharists (aka first communions), confirmations, and marriages.
If you are a parent recording your child's sacraments, please fill out your child's name under 'the elect' section and your information under 'parental information'. If you are an adult and filling out this form for yourself, there is no need to fill out the parental information.
